Consent Question

Dear Talkers,

I'd like to pose a consent question please. Can you tell me if you obtain consent for the following procedures in your facilities?
Dobutamine Stress Echocardiogram
Dobutamine Stress Nuclear Test
Adenosine Stress Nuclear Test
Lexiscan Stress Nuclear Test
Cardiac Computed Tomography Angiography (CCTA)

If so, is the consent obtained by the ordering physician or the cardiologist overseeing the test? Is a cardiologist available for each test, including the CCTA?

Thank you,
Traci Burcham

Have a question about filling in blanks on consents BEFORE the patient signs. In our system, the consent prints with a patient “label” on it. However, there’s also a blank for the patient’s name to be written onto the consent. Our staff is finding that the patient’s name blank is not being filled in, so the staff is writing the patient’s name in that blank so that the consent will be complete.

If the staff writes the name in that blank, does the staff member need to initial, date and time that entry?
